The form must be submitted after reading the rules of the sanatorium very carefully. All the rules and all subsequent changes thereto will be binding on all the guests staying in the premises. Only the guests whose names are mentioned above will be allowed to stay. Visit / Stay of any outside persons is strictly subject to the manager’s permission.

Instructions:

Application will be considered only after receipt of refundable deposit amount Rs. 6000/- and primary management expenses of Rs. 100/-

Application form to be filled by the applicant after carefully reading the rules to be followed by asking for the rule sheet. Application with incomplete information will not be considered.

Please note that maximum four persons will be allowed in one block. Children above three years will also be considered as an adult.

In any case, if the water tap remains open, Rs. 250/- has to be paid as fine.

It is necessary to bring a xerox copy of the Aadhaar card of the person making the booking and the person staying.
